The dimensions of the construction bite of the activator, as well as the positional changes of the mandible and hyoid bone, were studied on cephalometric radiographs in 30 subjects with Angle Class II, division 1 malocclusion. The cephalometric radiographs were taken at the start of treatment of each child, one with the mandible in the intercuspal position and one with the activator in the mouth. The activator patients were divided into two groups because of differences in the construction of the wax bite. In group 1 the wax bite separated the upper and lower incisors by 13 mm, while in group 2 the incisors were in an edge-to-edge position.
It was shown that in the incisor region the mean opening of the bite for groups 1 and 2 was 5.9 and 4.8 mm, respectively. A slight posterior rotation of the mandible (mean 2 degrees) was recorded. On average the pogonion point of the mandible moved downward and forward 5.3 and 3.7 mm, respectively, while the corresponding values for the hyoid bone were 1.0 and 1.8 mm, respectively. The condyle was displaced downward and forward 2.5 and 4.6 mm, respectively. A significantly larger sagittal displacement of the mandible (pogonion and condyle) was recorded in group 2 cases.
