A post treatment evaluation of multibonded lingual appliances in orthodontics

An evaluation was made of the use of multibonded lingual appliances in orthodontics, based on post treatment results of 10 consecutively treated cases with different malocclusions. Both extraction and non-extraction treatments were performed. Bonding was performed indirectly with prediluted Concise and treatment mechanics were similar to those used with labial appliances. The failure rate of the 145 brackets bonded was 2.8 percent. Pronounced carious white spots developed on two maxillary central incisors. The lesions were associated with incomplete filling in of adhesive between bracket base and enamel. Gingival swelling and general bleeding were scored after few months in seven patients with visible plaque around the appliances at every visit. The inflammation remained until the brackets were removed. Speech was impaired for a short period in seven patients and for the whole treatment period in one patient. Mean chair time associated with archwire change was 55 minutes (s.d., 12.0) and mean treatment time was 12.5 months (s.d., 6.0). It was concluded that multibonded lingual appliances may be a useful contribution to the orthodontist's armamentarium. However, the technique was found to be demanding, and correct bracket placement was judged as a key factor to reduced chair time.
